This questionnaire is a part of the research that I am doing for the book "TTC Dreams". This book will be for all women and couples that are trying to conceive and/or are dealing with infertility. If you would like additional information on this book, please visit the Book Submissions page on this site. This questionnaire will only be available through July 2007. At that time I will be contacting those that have filled out the questionnaire for further information and for permission to use their submissions.
